You received a 3-4 digit token via Email or Text Message
If you received an Email or Text Message from your practice with a 3-4 digit token, use this method to get access to your medical records.
- For User ID enter the email or phone number used to receive your Account Token. This will be your ID for all future sign-ins.
- For Password enter the Account Token that was sent to you.
- Click ‘Sign In’ & create a new password following the password requirements. Your setup for MyMedicalLocker™ is now complete!
